Admission Forms

The admission application has seven (7) components. All components must be received before an application is considered complete. You can use this application kit to print all seven forms with instructions.

Parent and Student Forms

First, complete these three forms and mail them with the $75 application fee.

❑  Parent statement
❑  Student statement (handwritten by student)

Recommendations and Transcripts

Next, the following forms should be printed and given or emailed to the applicant’s current school and teachers. After completion, the forms should be signed and mailed directly to the Kirby Admissions Office by the applicant’s school (not collected by the applicant). Provide a stamped and pre-addressed envelope with each request.
